How to Add and Configure Questions from Questionnaire Templates

In the KCM GRC Vendor Risk Management module, you can create questionnaires to send to the vendors or third parties your organization works with. When you create a questionnaire, you can use your own questions, or you can add questions from our questionnaire templates. Our templates are based on industry standards such as the Standard Information Gathering (SIG) questionnaire and the Consensus Assessment Initiative Questionnaire (CAIQ). Since these standards are familiar to auditors, you can use questions from our templates to expedite your vendor audits.

When you create a questionnaire, you can add a combination of questions from questionnaire templates and custom questions. For general information about creating questionnaires in your platform, see our How to Create and Configure Questionnaires article.

Note: The instructions in this article only apply to KCM GRC questionnaire templates. For information about custom questionnaire templates, see the Creating a Custom Questionnaire Template section of our How to Create and Configure Custom Questions in a Questionnaire article.

Adding Questions from a Questionnaire Template

When you create a questionnaire, you can add questions from the questionnaire templates we offer.

To add questions from a questionnaire template to a questionnaire, follow the steps below:

  1. Create a questionnaire. For instructions, see steps 1-4 of the Creating Questionnaires section of our How to Create and Configure Questionnaires article.
  2. In the Add Questions From section of the page, click the list button. When you click this button, the questionnaire templates we offer will display on the left side of the Questionnaire Templates window. Select questions from existing templates
  3. Click the drop-down arrow next to a template to view the sections in that template. Epand section in template
  4. Select the check boxes next to the questions you would like to add to your questionnaire. To sort through the questions in the template, you can either navigate through the pages by using the page numbers at the bottom of the window, or you can click through the template sections under the template name.  
    Tip: To add all of the questions in the questionnaire template to your questionnaire, select the check box above the list of questions. Then, click the Select all questions in "[Template Name]" button.
  5. After you finish selecting questions, click the Add button at the bottom of the window.  
Tip: After you have added your questions, you can view them by selecting the All drop-down menu in the Add Questions From section of the page. Then, click the name of the system template you added the questions from.

Configuring Questions from a Questionnaire Template

After you add questions from a questionnaire template to your questionnaire, you will need to configure points for each answer. When your vendors complete the questionnaire, the points that you assign to each question will be used to score their responses.

Your organization can determine a method for measuring the points that you assign to the questions. Each of a vendor's correct or partially-correct answers will contribute to the total score that the vendor receives for the questionnaire.

To configure a questionnaire that you created from a questionnaire template, follow the steps below:

  1. From the navigation panel in your platform, navigate to Vendor Management Questionnaires.
  2. From the Name column, select the questionnaire that you would like to configure.
  3. In the Questionnaire Versions section of the page, click the Configure button.
  4. Assign points to the questions in your questionnaire. You can either assign the same number of points to all questions or individually assign points to each question. For more information, see the list below:
    Note: The point values must be whole numbers.
    • To assign the same number of points to all questions, enter a point value into the field next to the Set For All button. Then, click the Set For All button.
      Important: If your questionnaire includes questions that use the checkbox answer format, the Set For All feature will also assign this number to the weight for each check box answer option.
    • To individually assign points to a question, click the pencil icon next to the question. Then, enter a number into the Points field and click the check icon.
  5. After you assign points to all questions and you are satisfied with your questionnaire, click the Mark as Configured button. When you click this button, your questionnaire will be in the Configured status.

Before you can send the questionnaire to vendors, the questionnaire will need to be marked as Reviewed. For more information, see the Reviewing and Finalizing Questionnaires section of our How to Create and Configure Questionnaires article.
