In your KCM GRC Vendor Risk Management module, you can create and configure questionnaires to send to the vendors or third parties your organization works with. If you would like to save configured or completed questionnaires for your records, you can export them as CSV files.

Exporting Configured Questionnaires
After you configure a questionnaire, you can export a CSV file that contains information about the questionnaire. Then, you can download the CSV file to your device.
To export a configured questionnaire, follow the steps below:
- From your navigation panel, navigate to Vendor Management > Questionnaires.
- From the Name column, select the questionnaire that you would like to export.
- In the Questionnaire Versions section of the page, click the Export button.
- Open the file download. If you would like, you can save the file to your device.
Information in the Configured Questionnaire CSV File
The CSV file will contain information about the questionnaire you configured.
For information about the headers that are included in the CSV file, see the table below:

If the question is from a system template, the fields in this column will display the name of the section that the question is from. If the question is a custom question that you added or imported into the questionnaire, this column will display Custom. |

The fields in this column will display each question in the questionnaire.
Important: Custom questions with answer options that are in the multiple choice or checkbox formats will have multiple rows in the CSV file. Each answer option for the question will include a row.

The fields in this column will display the type of answer option that the question uses, including Yes/ No/ N/A, multi for multiple choice, Free form for free form text, or checkbox for check boxes. |
| answer_options |
answer_options: The fields in this column will display each answer option available for the question. For information about the how answer options will display based on answer type, see the list below:

| is_correct |
The fields in this column will display the correct answer for the question. For information about how the correct answers will display based on answer type, see the list below:

| how_to_answer |
The fields in this column will display how the vendor should answer the question. For information about how the vendor should answer based on answer type, see the list below:

| score |
The fields in this column will display the total number of points a vendor can receive for the question.
Important: If the question type is multiple choice or checkbox, the score will be repeated on each row that exists for the question.

If the question is marked informational, the fields in this column will display TRUE. |
Exporting Completed Questionnaires
After a vendor completes a questionnaire, you can export a CSV file that contains information about the questionnaire, including the vendor's answers. Then, you can download the CSV file to your device.
To export a completed questionnaire, follow the steps below:
- From your navigation panel, navigate to Vendor Management > Vendors.
- From the Name column, select a vendor. When you select a vendor, you'll be taken to the Vendor Details page.
- Select the Assigned Questionnaires subtab.
- Click the Export button next to the questionnaire that you would like to export.
Note:The Export button will only display next to questionnaires that are in the Pending Review, In Review, or Reviewed statuses.
- Open the file download. If you would like, you can save the file to your device.
Information in the Completed Questionnaire CSV File
The CSV file will contain information about the questionnaire that the vendor completed, including the vendor's questionnaire answers.

| weight | This column only applies to checkbox questions. The fields in this column will display the weight of each answer option. For example, some options may have a weight of .5 points, while some might have a weight of 0 points. The weight for each answer option will be listed on separate lines within a row. |
| correct_answer |
The fields in the column will display the correct answer for the question. For information about how the correct answers will display based on answer type, see the list below:

| vendor_answer |
The fields in this column will display the vendor's answer to the question. For information about how the vendor's answer will display based on answer type, see the list below:

| max_score | The fields in this column will display the maximum number of points that the vendor could have received for the question. |
| current_score |
If you modified the original score that the vendor received for the question, the fields in this column will display the updated score. If you did not modify the original score, the fields in this column will be blank. |
| comments | If the vendor added a comment for the question, the fields in this column will display the comment. |
| issue |
If an issue was created for the question, the fields in this column will display the issue description. |
| issue_status |
If you created an issue for the question, the fields in this column will display the current status of the issue. The issue statuses are Open, Pending, and Closed. |
