Display the RAW URL and Decoded URLs in URL Tab

We would like the ability to see both the RAW URL that was displayed to the reporting user and the decoded URL in PhishER.

This allows us to more easily find the encoded URL in our Email Gateway and confirm that the URL was actually rewritten when it was displayed to the user (and not bypassed or exempted for some reason).
